Shiley™ Custom Tracheostomy Tube Service
Description
When a standard size tracheostomy tube doesn’t offer the right fit for your patient, you can turn to the Shiley™ Custom Tracheostomy Tube Service for help. You can modify our standard items to provide a custom fit for your difficult-to-fit neonatal, pediatric and adult patients.
Modifications include: - Longer or shorter tube shafts
- Additional sizes
- Customized cuffs
- Modified neck flanges: both style and angles
- Modified tube curvature and fenestration locations
Delivery Times - Sterile custom tracheostomy tubes: Approximately 4 weeks
- Non-sterile custom tracheostomy tubes: Approximately 7 business days
- All custom tracheostomy tubes are shipped by overnight delivery, unless otherwise requested
How to Order - All orders require completion of a Shiley™ Custom Tracheostomy Tube Template
- You can choose from three templates: Neonatal/Pediatric, Adult Single Cannula and Adult Dual Cannula
- Templates can be completed either on-line or off-line

- Offline - Click on the Ordering Templates links above to download an Adobe PDF template. Fill out the template as indicated, then either fax it to the number printed on the form or scan it and send it as a PDF to shileycustomtrach@covidien.com

Federal (U.S.) law restricts tracheostomy tube devices to sale by or on the order of a physician. By submitting a custom tracheostomy tube ordering template, you represent that the device is being requested on the order of a physician. Because these products are designed for a specific patient, custom tracheostomy tube orders cannot be cancelled or refunded once submitted.
